Prepare for Community Care
This is for agencies that are considering offering Community Care (Stacked/Shared/Short-Visit) but currently do not have a community that they serve. The objective is to be ready when the community asks you to come in, present and win the business.
Working together we will unlock the potential of Community Care for your agency. Our consulting service helps home care agencies identify community care opportunities such as independent living communities, +55 Active Adult Multi-Housing and +55 Active Adult Apartments, connect with them, and then tailor a home care proposal custom to their specific needs. From needs assessment to outreach strategies, we'll guide you in building a strong partnership proposal for effective community-based home care services.
Prepare for Community Care Package includes:
- Two 30-minute coaching sessions.
- Access to the CINCH Demonstration Communitty to be leveraged for presentations to communities.
- Webinars, documents, and marketing resources to build your understanding of these services and give you the ability show your potential partners how you will serve their residents!
